Association of the Expression of Cell Cycle Proteins p53, p14, p16, and Cyclin D1 with the Progression of Oral Squamous Cell Carcinoma in Taiwan

p53,p14,p16及cyclin D1表現和台灣口腔鱗狀細胞癌進展之相關性

摘要


口腔癌在台灣非常普遍。證據顯示口腔癌起因於一連續性,且多重性的基因突變,其中包括致癌森因與腫瘤抑制基因(tumor suppressor genes)之突變等。這些變化引起調節細胞週期蛋白的異常表現。本實驗的目的在研究細胞週期相關的p53, p14, p16,和CCND1蛋白表現和口腔癌病患的臨床參數之問的相關性。實驗的方法為利用免疫組織化學染色檢查73例口腔癌的p53, P14, P16,及CCND1蛋白表現。收集的數據被計算,並且用中t測試,專欄統計或者費雪測試評估。臨床的參數包括年齡,腫瘤大小,頭部淋巴結轉移與否及腫瘤分期。p值<0.05表示二組間具明顯的差異。實驗結果顯示p53, P14, P16和CCND1蛋白表現和患者的年齡、腫瘤大小或局部復發之間沒有明顯的相關。但p53及p16的表現和頸部林巴結轉移呈現負相關(P53, OR=4.36, 95%CI 1.53-12.39, P=0.0094) (P16, OR=4.98, 95% CI 1.02-24.32),而p16的表現也和腫瘤的臨床分期有明顯的相關(OR=4.18, 95% CI 1.26-13.88, P=0.0028)。由此研究的發現認為P53和p16的表現或許可以用來預測台灣口腔癌的進展。

Oral cancer is very common in Taiwan. Evidence suggested that oral cancer results from sequential, multistep gene mutations involving both oncogenes and tumor suppressor genes. These mutations cause abnormal expression of proteins regulating cell cycle proliferation. The aim of present investigation was to study the correlation between the expression of the cell cycle-associated proteins p53, p14, p16, and cyclin Dl (CCND1) and clinical parameters of oral cancer patients. The expression of p53, p14, p16, and CCND1 proteins was examined by immunohistochemistry in 73 oral squamous cell carcinomas. Collected data were calculated and evaluated with t-test, column statistics or Fisher's exact test. Clinical parameters included age, tumor size, neck lymph node metastasis, and tumor stage. A P-value of <0.05 was considered to indicate a significant difference. Results: There was no significant correlation between the expression of p53, p14, p16, or CCND1 and patients' age, tumor size, or local recurrence. However, there was a significant association of down regulation of both p53 (OR=4.36, 95% Cl 1.53-12.39, p=0.0094) and p16 (OR=4.98, 95% Cl 1.02-24.32, p=0.038) with cervical lymph node metastasis, and of down regulation of p16 with advanced tumor stage (OR=4.18, 95% Cl 1.26-13.88, p=0.0028). The finding of the present study indicates that p53 and p16 may be predictors of the progression of oral squamous cell carcinoma in Taiwan.
