以造袋術及矯正治療成功引導下顎含齒囊腫內之恆牙萌發－病例報告

發生在混合齒列期的顎骨囊腫並不多見，由於囊腫內或鄰近常有恆牙牙胚的存在，因此在治療囊腫時，如何成功保留恆牙牙胚，並使其順利萌發，成為牙醫師臨床治療上最大的挑戰。本文報告一例醫生於一位七歲女童右下顎的含齒囊腫，由於囊腫內包含右下第一及第二恆小臼齒之牙苞，我們決定藉由造袋術來治療囊腫，經過一年半重複沖洗及合併矯正治療，我們不但順利成功治癒囊腫，而且保留囊腫內之恆牙牙胚，並引導其萌發至理想咬合位置。此囊腫迄今追蹤七年，無復發跡象。本報告顯示針對影響到未萌發恆牙牙胚之下顎骨含齒囊腫，以造袋術配合矯正治療，可達到很好的囊腫治癒效果，同時也可保留恆牙牙胚，並使其順利自行萌發至理想咬合位置，此臨床經驗值得牙醫師於混合齒列期治療下顎骨含齒囊腫時，選擇治療方法之參考。

Jaw bone cysts are rarely seen during the stage of mixed dentition. Because the permanent tooth germ is usually close to or included in the cyst, determining how to preserve the tooth germ and guide it to erupt in its proper position is a big challenge for the dentists. In this article, we report a large dentigerous cyst in the mandible of a 7Wear-old girl. Because the dentigerous cyst included 2 tooth germs of the right mandibular first and second permanent premolars, we decided to use marsupialization to treat this cyst. After repeated irrigation of the cyst lumen and orthodontic treatment for 1.5 years, we successful cured the dentigerous cyst, preserved the 2 tooth germs, and further guided the 2 premolars to erupt in their final proper occlusion positions. The patient was followed up for 7 years and no recurrence of the cyst was found. We suggest that marsupialization combined with orthodontic treatment can successfully treat a dentigerous cyst, preserve the associated tooth germs, and subsequently guide the teeth to erupt in their idem occlusion position. This clinical experience is a good reference for dentists for selecting the best treatment method for a mandibular dentigerous cyst in the stage of mixed dentition.
