Kaposi's Sarcoma in a Heart Transplant Recipient: A Report of a Single Case in Taiwan and Review of Literature

卡波西氏肉瘤發生在心臟移植後的病人-一病例報告及文獻回顧

摘要


38歲男性,因心肌病變合併鬱血性心臟衰竭接受心臟移植手術。出院後服用免疫抑制劑治療,爾後,追蹤心內膜切片顯示輕微性排斥。心臟移植約一年後,在病人腹股溝的皮膚及淋巴結處,發現卡波西氏肉瘤(Kaposi's sarcoma),進一步電腦斷層檢查發現在氣管旁右側(right-side of paratracheal)及前縱隔腔(anterior mediastinum)有淋巴結腫大。因為目前卡波西氏肉瘤發生在心臟移植後的病例,在世界英文文獻上報告極為罕見,且其預後大多非常差(平均在診斷後2-6月死亡)因此提出報告及文獻回顧,供學者參考。

Kaposi's sarcoma is one of serious complication in heart transplantations. We present a 38-year-old Taiwan man who received heart transplantation due, to ischemic cardiomyopathy. Kaposi's sarcoma was found in the patient's inguinal skin and lymph nodes twelve months after heart transplantation. After reduced the dosage of immunosuppressive drugs including cyclosporine, azathioprine, and prednisolone, the skin lesion of kaposi's sarcoma was disappeared and without other new skin lesions. He was survival for more than 4 years after transplantation. Herein, we present a very successful heart-transplant case and suggest a principle of management.
