摘要

耳廓之惡性汗腺瘤（malignant eccrine spiradenoma）是外耳極為少見之腫瘤。本科於民國86年經歷一例惡性汗腺瘤，患者為78歲男性，左側耳輪原有一個良性腫瘤有50年之久，主訴於就診前一個月，該腫瘤開始有紅腫潰瘍之惡性變化，易流血，雖每日敷藥，不僅無法痊癒，且有逐漸擴大之趨勢。於全身麻醉下，進行腫瘤切除手術，將整個腫瘤及其附近組織以1cm之安全界限做楔狀切除術（wedge excision）後縫合，術後輔以放射線治療，5個星期內照射5000 cGy，門診追蹤半年，傷口癒合良好，無復發跡象。

The malignant eccrine spiradenoma of the auricle is a exceedingly rare neoplasm. We incidentally experienced a case of malignant eccrine spiradenoma in Oct. 1997. The patient was a 78-year-old male. He suffered from a protruding tumor of L't auricle for 50 years. Unfortunately, the tumor had a malignant degeneration and became a nonhealing ulcer with easy-contact bleeding, about 1.5X1.2X0.5cm in size for one month. After receiving the operation of wedge excision with 1 cm safety margin, radiotherapy was performed with 5000 cGy in 5 weeks, he is uneventful for half a year without recurrence.
