Adenocarcinoma Arising from Mature Cystic Teratoma of the Ovary

Malignant transformation associated with mature cystic teratoma (MCT) is uncommon. The most common malignant tumor is squamous cell carcinoma. Adenocarcinoma arising in MCT is extremely rare. A 66-yearold woman suffered from abdominal fullness, diffuse tenderness, vomiting, constipation, consciousness disturbance, and fever for 4 days. Ileus and pelvic mass were noticed. Due to suggestion of bezoar with intestinal obstruction, she underwent an exploratory laparotomy. During operation, a huge ovarian mass on the left with cystic degenerative changes was found, and oophorectomy was performed on the left. Results of histopathological examination and immunohistochemical studies showed that the ovarian tumor was a benign mature cystic teratoma with adenocarcinoma from gastrointestinal epithelial origin. Postoperative recovery was uneventful for 12 months. We present this uncommon case with clinically distinctive manifestations and review reports in the literature.
