Intra-Articular Giant Cell Tumor of Tendon Sheath of the Knee

The giant cell tumor of the tendon sheath (GCT-TS) is a benign growth, which rarely occurs intra-articularly. In this report, we describe a case of intra-articular GCT-TS in the right knee joint. Clinically, the patient presented to our orthopedic clinic with anterior knee pain, accompanied by a palpable mass in the peri-patellar region. Contrast-enhanced computed tomography showed an enhanced soft-tissue nodule located in the lateral recess of the patellofemoral joint. From MR study, the mass appeared intermediate to slightly high in signal intensity on T1-weighted images, and slightly high on T2-weighted images. Some dark dots were seen in this lesion on T2-weighted images, corresponding to areas of hemosiderin deposition as proved by histological findings. The final diagnosis of GCT-TS was made from evidence of typical histological findings revealing giant cells and hemosiderin-laden foam cells within a matrix of ovoid to polygonal cells. In this case report we stress that intra-articular GCT-TS should be included in the differential diagnosis whenever a solitary intra-articular nodule, which is enhanced on CT study and characterized by dark-dot foci inside the nodule on MR images, is encountered in patients presenting with anterior knee pain and a palpable mass.
