A Different Presentation in Hepatic Schistosoma Japonicum Infection

Schistosomiasis has been mentioned as ”great neglected disease of mankind”. The prevalence is quite different among schistosomiasis. Most reported cases of Schistosoma japonicum in Taiwan are immigrants from endemic areas, especially China. Periportal fibrosis is specific to schistosomiasis and can be seen on ultrasonography and computerized tomography. We reported a 61-year-old male who complained of epigastric pain and chillness for 3 weeks. The computerized tomography of abdomen only revealed fibrotic bands of liver. The liver pathology only revealed periportal fibrosis without schistosoma granuloma. These presentations were rare in hepatic S. japonicum infection. As there are more travelers and immigrants from endemic areas to Taiwan, clinicians should be aware of hepatic S. japonicum infection as an important pathogen of liver diseases.
