Synchronous Adenocarcinoma of the Appendix and Transverse Colon Associated with Pseudomyxoma Peritonei Presenting as an Acute Appendicitis

Pseudomyxoma peritonei (PMP) is an unusual malignancy characterized by abundant intraperitoneal mucinous and gelatinous material associated with an intraperitoneal adenocarcinoma. It is usually arising from the goblet cells of the large bowel and appendix. Even with improvements in our understanding of PMP and with improvements in imaging modalities, definite preoperative diagnosis and treatment selections remain difficult. The treatment of choice is surgical removal of the mucinous collection and a resection that is as complete as possible. Systemic chemotherapy, intraperitoneal chemotherapy and radiotherapy have been tried and have been shown to have limited value. The overall survival rate is 40-50% at 5 years and 20-40% at 10 years, however, long-term survival has been reported. Herein, a case of combined cancers of the appendix and transverse colon with PMP presenting as acute appendicitis is described.
