Radiofrequency Sinus Excision: A Better Alternative to Marsupialization Technique in Sacro-Coccygeal Pilonidal Sinus Disease

Background: While the disease entity has long been known, surgeons have not reached consensus on the best therapy for pilonidal disease. This prospective, randomized study compared the outcomes of the excision and marsupialization technique and the sinus radiofrequency excision technique. Materials and Methods: Thirty patients with chronic pilonidal sinus disease were randomized to undergo either a radiofrequency sinus excision (n=15) or excision and marsupialization (n=15). An Ellman radiofrequency generator was used for the first procedure. The demographic data, postoperative results, complications, and recurrence were recorded. Patients' satisfaction with the outcome of the procedure was also evaluated. Results: The operation time, postoperative pain, hospital stay, and off-work periods were significantly less (p<0.05) in the technique employing radiofrequency surgery. At 2-year follow-up, 1 patient from each group had a recurrence. The patients from the radiofrequency group expressed a greater satisfaction in comparison to their counterparts. Conclusions: This pilot study shows that radiofrequency sinus excision is a simple and safe procedure for pilonidal sinus disease. It requires a shorter hospital stay with reduced postoperative pain and allows earlier resumption of work.
