Combined Excimer Phototherapeutic Keratectomy and Hyperopic Photorefractive Keratectomy for Reis-Bücklers' Corneal Dystrophy

This report describes a patient with Reis-Bücklers' corneal dystrophy who underwent simultaneous treatment by excimer phototherapeutic keratectomy (PTK) and hyperopic photorefractive keratectomy (PRK). The 48-year-old male had experienced episodes of bilateral ocular pain and progressive blurred vision since his teenage years. On examination, the refraction error was +4.25-1.75×130 in the right eye and +4.00-1.50×70 in the left eye. The best-corrected visual acuity was 6/20 in the right eye and 6/12 in the left eye. Slit-lamp examination revealed a diffuse, honeycomb-shaped corneal opacification in the region of Bowman's layer and varying degrees of corneal epithelial erosion in both eyes. A positive family history was evident in his two daughters, whose vision was unimpeded. The extent of corneal opacification decreased after excimer PTK and hyperopic PRK. One month after surgery, the refraction error was +2.50-1.75×5 in the right eye and +2.00-0.75×60 in the left eye, and the best-corrected visual acuity was 6/7.5 in the right eye and 6/8.6 in the left eye. This improved visual acuity occurred without evidence of corneal erosion at a 10-month follow-up. The combination of excimer PTK and hyperopic PRK may provide a satisfactory therapeutic alternative in patients with Reis-Bücklers' corneal dystrophy and recurrent corneal erosion.
