Gastric Outlet Obstruction Caused by a Giant Gastric Adenoma: Report of a Case
巨大胃腺瘤引起之胃出口阻塞：病例報告
摘要
胃乳突狀腺瘤很少見且在臨床上常是偶然發現。大部分病人皆無症狀，但建議密集的追蹤以防止胃癌的發生。一般而言，胃乳突狀腺瘤不大。巨大胃腺瘤引起的胃出口阻塞極爲罕見。我們報告一位八十四歲女性，臨床症狀爲漸進性吞嚥困難約半個月。影像學檢查發現有一巨大的胃腫瘤約12×10×8公分。病人接受部分胃切除，術後病理檢查爲腺瘤。總之，正確的診斷和仔細的治療計畫在由胃腺瘤引起之胃出口阻塞的處置上扮演相當重要的角色。

Gastric papillary adenoma is rarely encountered and is usually an incidental finding in daily clinical practice. Most patients with such a tumor are asymptomatic, but should be closely followed up because of the risk of malignant transformation. Gastric adenoma is usually small, and it is exceptionally rare that a giant gastric adenoma causes gastric outlet obstruction. We describe an 84-year-old woman who presented with progressive dysphagia for half a month. An image study revealed a large gastric tumor measuring 12×10×8 cm in size. Partial gastrectomy was performed and pathological examination showed a papillary adenoma. In conclusion, accurate diagnosis and a careful treatment plan are important in the management of gastric outlet obstruction caused by gastric adenoma.
