Severe Labial Fusion as a Cause of Voiding Dysfunction in a Young Woman: Report of a Case
陰唇黏合導致一年輕女性排尿功能障礙：病例報告
摘要
在我們的病例報告中，一名21歲未婚女性，沒有外傷或尿路感染病史。因爲下腹悶痛及月經量少而在婦產科求診，腹部超音波未有子宮或卵巢異常之發現，近半年來，有排尿日漸困難之現象，排尿時問延長且須要用力壓迫腹部，因而轉介至泌尿科求助。經由評估，靜脈腎盂照影顯示泌尿系統無結構上異常，進一步安排尿路動力學檢查時，由於尿管置入困難，發現患者有陰唇融合之現象，在使用雌激素軟膏後，症狀持續而未緩解，所以接受陰唇切開重建手術，患者於術後排尿症狀完全緩解，也同時改善了月經疼痛腹脹之症狀。此個案提醒我們對於排尿障礙之病患，詳細的理學檢查可以幫助早期正確的診斷，進而避免許多侵襲性檢查。

We describe a 21-year-old female who suffered from voiding difficulty, incomplete voiding sensation and a small-caliber urinary stream for 6 months. She had menarche at 12 years of age, and denied any history of trauma, sexual abuse, intercourse or genital tract infection. Her uterus and both ovaries were anatomically normal as evidenced by abdominal sonography. Intravenous pyelography showed a normal urinary system. Uroflowmetry showed a flattened curve with 2 ml/sec of maximum flow rate (Qmax). While attempting to catheterize her bladder for urodynamic study, severe labial fusion with a pinpoint opening was mistaken for urethral orifice. She received two weeks of local premarin cream application but the symptoms persisted. After labial reconstruction, she has had no more voiding dysfunction.
