Angle Blade Plate Fixation for Distal Femoral Fracture - A Personal Clinical Experience

以成角骨板固定股骨遠端骨折之臨床經驗

從一九八六年一月起到一九八八年六月,作者以手術整復及AO的九十五度成角骨板固定十八位病人的遠端股骨骨折。其中十四位為男性,四位為女性。年齡由十八歲到八十二歲,平均為四十八歲。本研究根據AO的分類將骨折分類,其中十一位屬A2型,六位為C2型,一位為C3型。手術後追蹤的時間從六個月到三十六個月不等,平均為二十一個月。十七位病人在六個月內骨折已癒癒合,另一位在一年內也癒合。研究中無感染,骨板斷裂,及癒合不正之病例。十六位病人可以將膝關節彎曲超過九十度。十六位病人最後的結果為極優或良好(占百分之八十八),兩位三結果評為失敗。雖然AO的手術技術比較複雜,但是臨床結果相當好,還是值得採用。

From Jan 1986 to June 1988 eighteen patients with distal femoral fracture were treated by open reduction and internal fixation with AO 95 degree angle blade plate. Fourteen patients were male and four were female. The age was from 18 to 82 yeas (average 48 years). The fracture patterns were classified according to the AO classification: 11 patients in type A2, 6 in type C2 and 1 in type C3. The follow-up period ranged from 6 months to 36 months (average 21 months). Seventeen patients obtained solid union within 6 months and one at the end of one year. No infection or implant failure occurred. Sixteen patients could flex the knee more than 90 degrees. Excellent or good functional and radiological results were achieved in sixteen patients (88%). Though the AO tech-nique was complex, the excellent results made it worth.
