Biomechanical Study of Cortical Allograft in Canine Fibular Model

異體皮質骨移植於狗腓骨模型的生物力學研究

摘要


自從冷凍技術成功降低免疫反應後,同種(異體骨)在骨科領域已廣汎被使用,而臨床結果郤很難預測訐估,本實驗目的是想藉狗腓骨骨移植模型了解同種骨移植後生理過程和生物力學的表現關係,三十隻成狗被選用在兩側排骨各截取三公分缺捐,右邊用冷凍同種骨,左邊用新鮮同體骨補齊缺捐亚分三組分別養四、八、十二週,之後取腓骨標本做扭力測驗。  最後餘二十五隻狗的結果做評估,顯示十二週內並不如預期的完全,因此所測出的扭力值較偏差,無法顯出同種骨與同體骨的差異,但從其他指標如X光癒合形態和生物力學表現分級情形來看,兩者結果仍顯示差異,同種骨梢遜於同體骨,意味著殘留小量免疫反應的潛在威脅,將來對同種骨的加工去除殘餘的免疫性的努力可望能改善同種骨臨床應用的成積。

The allograft transplantation become widely accepted since the cryopreservation of the allograft is believed to reduce its immunogenecities. Howevewr, the clinical results are still unpredictable, therefore we attempted to study the biological healing of the allograft corresponding to its biomechanical performance from an animal model. Thirty adult dogs were included in this study. Fibular segment of 3 cm in length was selected for the graft which replaced the defect created by double osteotomy in the middle portion of the fibular bone. One side was replaced with deep frozen allograft and the other side with fresh autograft. The follow up time was 4, 8 and 12 weeks separately in three divided groups. Regular monthly radiographical evaluation and final torsion test after harvest were performed in each animal. Twenty-five dogs completed the study. It showed that the healing of the graft was not complished in 12 weeks. Thus the measured torgue strength were found insignificantly different between the autograft and the allograft. From the other parameters such as rediographical typing and the biomechanical staging according to the failure patterns, we could draw the impression that there were some difference between the two kinds of the grafus. The allograft is inferior to the autograft probably due to the potential risk of residual immunogenecities. The further reserches to reduce the immune response to improve its final result may be promisingfor theclinical application of the allograft.
