The Medial Antebrachial Cutaneous Nerve as an Autograft Donor for the Repair of Digital Nerve Defect

摘要

採用自體神經移植來做手指較大距離的神經缺損手術已有一百年以上的歷史，許多不同來源的神經均被使用過，例如：腓腸神經、後骨間神經、外側前臂皮神經、及內側前臂皮神經等。
在過去三年中我們經歷了七個病人的八根手指，共有十一條神經接受內側前臂皮神經經移植修補手術，手術後病人追蹤10到52個月(平均24.8個月)其中六個病人的十修神經二點間距離分辨到5到10公厘，七個病人中只有一個有神經瘤的產生，一個病人有輕微的蟹足腫形成，但二人均未有任何不適的感覺。
與其他神經比較，內側前臂皮神經解剖位置相當固定，很容易取得，且手術只局限於一側上肢，手術拼發症相當少，且手術疤痕隱藏在手肘內側不易為人發現，它的形狀與直徑也與手指神經相近，因此我們認為用內側前臂皮神經來條補手指神經的缺損有令人滿意的結果。

Autogenous nerve graft for a large gap of digital nerve had been used for more than 100 years. Many sources of nerve have been used before such as sural nerve, posterior interosseous nerve, etc.
In the last five years there were 7 patients with 11 digital nerves on 8 digits which were repaired with medial antebrachial cutaneous nerve graft. The patients were followed up from 10 mouths to 52 months (average 24.8 months). Ten nerves out of 6 patients regained two-point discrimination between 5 and 10 mm. the morbidity of donor site is minimal. One of the 7 patients had a neuroma development at donor site, another one had keloid formation. But both of them have no complaint about that.
In comparison with other nerve as a graft for digital nerve, the medial antebrachial cutaneous nerve is easy to take. The surgery is limited to one extremity only, it is unnecessary to do another anesthesia or incision on the lower extremity. The shape and diameter of the medial antebrachial cutaneous nerve corresponds to those of digital nerve quite well. The ratio of nerve to connective tissue is similar to that of sural nerve. In our experience the medial antebrachial cutaneous nerve served a good autogenous donor graft for digital nerve repair.
