Surgical Treatment of Intertrochanteric Fractures of the Femur

Three hundred and ninety-six intertrochanteric fractures of the femur were treated with a hip compression screw plate for internal fixation at Chang Gung memorial Hospital, Kaohsiung from January 1986 to December 1991. Two hundred and forty patients received follow-up study for an average of twenty months. According to Evan’s modified classification, there were 92 stable fractures (38%) and 150 unstable fractures (62%). The degree of osteoporosis was graded according to Singh’s index revealing 134 fractures of grade 3 or lower (55.4%). Ninety-one percent of the patients experienced satisfactory results. The union rate was 97% and complication rate was 8.7%. The complication rate was 4.3% for the stable fractures and 11.3% for the unstable fractures. Complications included having the screw cutting out of the femoral head, lag screw penetrating into the joint, wound infection, avascular necrosis of the femoral head and malposition of the side plate. The causes of screw cutting out included severe osteoporosis, unstable fractures and poor position of the nail placement. The hip compression screw plate offers the advantage of good fracture healing, early ambulation, a high rate of satisfactory results and a low complication rate. The hip compression screw plate is a simple and effective method of internal fixation for intertrochanteric fracture of the femur.
