Dome-Shape High Tibial Osteotomy for Osteoarthritic Knee a Seven to Nine-Year Follow-Up Study

A total of 18 patients (23 knee joints) had received high tibial osteotomy with the help of the cutting jig designed by the author and had been followed up for more than seven years at least and an average of eight years. They included six men and twelve women whose average age at operation was fifty-eight years. They had an average body weight of 63 kg and height 159 cm. Preoperatively, Hospital for Special Surgery (HSS) knee score, mechanical angle and patellar tendon length and patellar length (TL/PL) ratio of each patient were evaluated. The average preoperative HSS knee score was 68 and was improved to 88 postoperatively. Ninety-two percent of the patients had an excellent or good results. One knee was changed to total knee replacement three years after the osteotomy because of a poor result. The average valgus correction was twelve degrees. Apart from loss of valgus correction in one case, there was no complication like peroneal nerve palsy, infection or non-union. The preoperative TL/PL ratio was 0.88 ± 0.15. It was 0.90 ± 0.19 at the time of follow-up. The difference between the two was statistically insignificant (p>0.5). Patellar infera did not occur postoperatively. Five patients had a body weight 1.32 times greater than the ideal body weight. Three of them had received total knee replacement in the contralateral knee.
