From July 1984 to Dec 1993, 145 patients with 146 subtrochanteric fractures were treated with dynamic hip screw devices. This injury was the result of high-energy trauma in 80.0% of cases. Most cases were treated with dynamic hip screw devices alone, and also with adjuvant bone graft in six cases. There were 1 case of wound infection, 1 case of fat embolism and 2 cases of pneumonia. Two patients died during hospitalization,and three died before fracture union.There were 140 cases followed up for periods ranging from periods ranging from 8-96 months. The average time for union was 5.7 months (range: 3.5-10 months). Nonunion was found in 10 cases, and implant failure in 8. These complications are common in type Ⅳ and ШA fractures. The union rate was 87.2%. These results suggest that the dynamic hip screw device is one of appropriate methods of treatment in cases of subtrochanteric fracture, and that special attention should be given to types Ⅳ and ШA fractures.
