Surgical Reduction of Acute Complete Acromioclavicular Joint Dislocation (A Report of A Technique)

From Jan. 1992 to July 1995, we report 39 cases of acute complete A-C joint dislocation treated surgically at Cardinal Tien Hospital. The purpose of the study is to report the results of surgical reduction of A-C joint dislocation using a modified technique of surgical repair involving the articular capsule and the superior AC ligament, imbrication of deltoid-trapezius muscle aponeuro sis in addition to the coracoclavicular circleage wire loop fixation. Patient age ranged from 23 Y.O. to 52 Y.O.; Follow-up time from 12 mos. To 42mos.. Using the Imatani evaluation system, the overall results were satisfactory in 90.7%; with excellent result in 28 cases, good result in 9 cases, fair result in 2 cases. Five cases with minor post-op complications. It is our recommendation that meticulous surgical repair of the supraclavicular soft tissue layers around the distal clavicle end be supplimented with C-C circleage wire loop fixation which maintained the physiologic scapulocavicular synchronous rotational motion of the A-C joint,and ensure a stable anatomically reduced A-C joint.
