Ipsilateral reversed vascularized fibular bone graft has been successfully utilized in the treatment of a massive tibial plateau defect at our hospital. A forty-year-old man suffered from a comminuted fracture of the tibial plateau in a motor vehicle accident 24 months ago. The fracture was treated initially with open reduction and internal fization with plate and screws. Wound infection developed postoperatively which resulted in severe osteomyelitis. Repeated debridements following removal of implant resulted in an open defect measured 10x10 cm in size, and 6 cm in depth, in the proximal tibia. Ipsilateral reversed vascularized fibular bone graft of 12 cm in length was harvested and fixed into defect with screws. Ilizarov external skeletal fixator (ESF) was applied for stabi-lizing the affected limb. Hypertrophy of the fibular graft with complete union of the defect was noted at 22 months follow-up and the patient regained satisfacto-ry functional recovery from the injured extremity.
