Safe Zone for Application of Transpedicle Body Augmenter

PURPOSE The study quantifies dimensions of thoracic and lumbar pedicles to determine potential safe zones for vertebral augmenter posterior reconstruction of collapsed vertebral bodies. METHODS T1-L5 segments from two human dry bones were separated into individual vertebra. Morphometrical calibration was done on dry bones and corresponding radiograms. Digital radiograms of thoracic and lumbar spines from 105 healthy subjects were obtained to analyze the pedicle and vertebral body relationship. Medial-lateral width and cranial-caudal height of pedicle and body were determined in anterior-posterior radiograms. The safe-zone width was defined as medial pedicle cortex to body lateral cortex, and height as caudal margin of pedicle to body cranial margin. Potential restoration ratio was defined as safe-zone height divided by anterior body height. RESULTS Dimensions varied with individuals and levels, but not between the left and right pedicles. The range of safe zone width was 5-7mm for T1-T8, 8-9.5mm for T9-L3 and 9-10mm for L4-L5. The range of safe zone height was 7-10mm for T1-T8, 13-20mm for T9-L3 and 15-17 mm for L4-L5. The potential restoration rate was 35-50% for T1-T8, 55-70% for T9-L3 and 51-55% for L4-L5. CONCLUSIONS The study provides the anatomic basis for the new technique of transpedicle body augmenter. The potential restoration rate and the concept of safe zone provide the references for surgical plan.
