Vascularized Iliac Bone Grafting for Non-Union of Femoral Neck Fracture in Young Patients-A Mean 10-Year Follow up

Introductions: Treatment in non-union of femoral neck fracture in young patient is complex. The estimated non-union and aseptic necrotic rate are notable. We presented the mid-term outcome of internal fixation combined with iliac bone grafting. Materials and Methods: From 1991 to 1998, open reduction and pinning combined with vascularized iliac bone grafting were performed on 14 patients with non-union of old femoral neck fracture. The mean delay of treatment was 8.6 months. Patients ranged in age from 18 to 44 years (mean, 26.2 years), and the mean follow-up was 124.6 (range, 103-158) months. Results: Bone union was achieved in all patients. Head collapse within 1 year was noted in two patients, one of whom eventually had hip arthroplasty in the same year. The average limb lengthening in the remaining cases was 1.5 cm and the mean Harris Hip Score was 93 (range, 89-97). The last radiographs did not show any progression of arthritic change. Conclusions: Our series yielded an encouraging result of vascularized iliac bone grafting in treating the non-union of femoral neck fracture in young patients. The mean 10 years follow-up did not show avascular necrosis of head or degenerative process of the hip joint.
