Alterations of Glomerular Levels of Glutathione Peroxidase in Diabetic Nephropathy

To investigate the status and the role of glutathione peroxidase (GPX) in diabetic nephropathy, we have measured the plasma and urinary GPX levls in 14 patients with diabetic glomerulosclerosis (DGS), and measured the glomerular GPX immunostaining in these patients and those of rats with different duration of diabetes, using the immunohistochemical .method. The results demonstrates that the plasma GPX level was significantly lower in DGS patients than in either diabetic patients without nephropathy )p<0.05) or normal controls (P<0.01). The urinary GPX level was also significantly lower in DGS patients than either diabetic patients without nephropathy or normal controls (both P<0.05). The immunostaining score of glomerular GPX was significantly lower in DGS patients than that of normal controls (p<0.05), and was negatively correlated with the glomerular sclerosing score and the index of mesangial expansion. Serial examination of glomerular GPX showed that the immunostaining score of glomerular GPX were significantly lower than normal contols in diabetic rats after induction for one and three months, respectively, and the staining score was also significantly lower in diabetic rats of three months old than that of one week old. In conclusion, this study demonstrate that all the plasma, urinary and glomerular GPX level are decreased in DGS patients, and the immunostaining of glomerular GPX decrease progressively with the progression of disease in diabetic rats. Our results indicates a decreased antioxidant defense in the diabetic kidneys.
