Mycoplasma Pneumoniae Pneumonia Associated with IgA Nephropathy and Rapidly Progressive Glomerulonephritis

A 21-year-old man developed mycoplasma pneumoniae infection and acute renal failure. Renal biopsy showed mesangial depositions of IgA, C3 (complement 3) with crescent formation and global sclerosis. Attemps to demostrate mycoplasmal antigen failed. Treatment with steroid, plasmapheresis and 5-days of cyclophosphamide were administered but the renal function remained poor. We suggest that in some cases, IgA nephropathy may follow mycoplasma pneumoniae infection which induces immune-complex reaction. The crescent formation in IgA nephropathy of this patient may be also linked to mycoplasma pneumoniae infection and poor renal outcome.
