Hidden Cause of Malnutrition in a Patient on Peritoneal Dialysis

Extrapulmonary tuberculosis (TB) - especially head and neck TB lymphadenitis - is more common in dialysis patients than in the general population. Herein, we present a patient on peritoneal dialysis for eight months with new-onset malnutrition, anemia and decreased dialysis efficacy over a period of two months. Low-grade fevers were also noted. A whole body inflammation scan showed a suspicious mass in the right subclavian fossa, which was excised and found to contain granulomas with Langerhans giant cells. Although the acid-fast bacillus stain, TB polymerase chain reaction, and culture were negative, her clinical picture improved dramatically after anti-TB treatment. Clinicians should be aware of the clinical presentation of TB lymphadenitis in new patients on dialysis.
