本研究的目的是要探討護理之家住民對日常活動的主觀感受。以半結構式訪談方法,訪問了十二位居住於某醫院附設之護理之家的老年住民。分析訪談結果顯示,老年住民們對於機構中制式的生活作息及照護模式大多感到不習慣。但是住民們多能體諒這是團體生活,不可能依照每個人的喜好來進行。住民的調適結果是降低對生活的要求與期待,不只是基本日常活動的參與變得被動、依賴,也沒有興趣參與其他的活動。因此建議護理之家等長期照護機構應提供以住民為中心的服務,尊重住民的意願與感受,並由職能治療師設計適合住民的照顧計畫與日常職能活動,透過幫助住民參與有意義的活動來維護住民的價值感、凝聚感與身心功能,讓其能夠擁有一個有品質的晚年生活。
The purpose of this study was to investigate the nursing home residents' perspectives about their daily occupations. Twelve elderly residents of a hospital affiliated nursing home were interviewed by following a semi-structured interview guide. The results showed that most of the residents experienced difficulties getting used (adjusting themselves) to the routinized living schedules and care pattern. However, they agreed that it was impossible to address individuality in a group residential setting. They adapt to this live style by lowering their demands and expectations of life. As a result, they became not only passive and dependent to basic daily activities but also reluctant to participate any other activities. We suggested to provide a resident-centered care plan designed by occupational therapists congurent with the residents' volition, feelings and abilities for nursing home facilities. By motivating the residents to participate meaningful activities, their sense of worth and coherence, physical and mental functions can be maintained, thus ensuring them a late life with satisfactory quality.
