等速肌力測試:重力效應之量化與速度對重力效應之影響

Quantification of Gravity Effect Including Speed Consideration during Isokinetic Testing

摘要


本文目的係在收集一般人膝學關節肌肉所負擔的最大重力效應值,並精簡重力效應量化的方法;進而了解在等速肌力測驗中速度對重力效應的影響,藉以與速度對最大肌力的影響程度做比較,並探討重力效應在不同速度下對測得肌力之影響程度。本文係採三十位年輕健康男性,分別利用人體測量學實臉,Cybex Ⅱ(上標 +)雙頻記錄器,與Cybex Ⅱ(上標 +) 電腦等三種方式測量膝關節肌肉最大重力效應值;並利用Cybex Ⅱ(上標 +) 測量膝伸肌與膝屈肌之最大肌力以茲比較。結果發現:一、以人體測量實驗決測得之最大重力效應值為7.57±0.32呎磅,是膝伸肌最大肌力之5.6%-11.4%、膝屈肌最大肌力之10.7%-17. 6%,其中速度愈快比例愈高。二、不同速度測得之最大重力效應值相互間呈現顯著差異性;且速度愈快,最大重力效應值愈小。三、以CybexⅡ(上標 +)雙頻記錄器於高速(210°/sec-270°/sec)時測得之最大重力效應值最接近人體利量實驗值。以Cybex 電腦到之則於慢速(30°/sec)時測得值較接近人體測量實驗值。

Gravity effect (GE) acts as a key role in manual muscle testing. In fact, the importance of GE is magnified if subjects generated low torque due to acceleration, muscle fatigue, or disability. Failure to consider GE during isokinetic endurance test, fatigue test, or high velocity test at knee joint greatly underestimates active knee extensor muscle torque and overestimates active knee flexor muscle torque. The purpose of this paper is to quantify gravity effect and to find the influence of speed on gravity effect during isokinetic testing. Thirty healthy young men performed isokinetic testing at 60°/s, 120°/s, 180°/s, 240°/s, 300°/s, and attended weigh limb procedure with Cybex Date Reduction Computer (CDRC), and Dual Channel Recorder (DCR) at eleven different speed (12°/s, 30°/s, 60°/s, 90°/s, 120°/s, 150°/s, l80°/s, 210°/s, 240°/s, 270°/s, 300°/s). Anthropometric method to collect limb weight is also applied by measuring leg volume for each subject. The finding were (1) Maximal gravity effect resulted from laboratory method is 7.57±0.32 ft 1b. and is found to be similar to the value measured by DCR at higher velocity (210°/s to 270°/s). (2) The percentage for peak gravity effect to peak corrected muscle torque is various depending on testing velocity from 5.6% to 11.4% for knee extensor and 10.7% to 17.6% for knee flexor; and the faster the testing velocity, the higher the percentage. (3) Peak gravity effect measured by Cybex machine (CDRC or DRC) is various depending on testing velocity: the faster the velocity, the lower the value; and the phenomenon is more obvious by using CDRC. It is suggested to measure maximal gravity effect at 30°/s by using CDRC, and at high velocity (210°/s to 270°/s) by using DCR in clinical application.
