臨床膝部短波熱效應之研究

Clinical Heat Effect of Shortwave Diathermy on Knee Joint

摘要


This is a pilot study to observe the different effect of shortwave diathermy by using coil cable and drum applicator on knee joint. All of 10 study subjects are physical therapy students. They do not have any musculoskeletal or neuromuscular disorder around their lower extremities. The average age of them is 21.8±0.5 y/o; the average height of them is 170.6±3.7cm; the average weight is 69.4±2.4kg. All of their right knees reveived shortwave diathermy twice individually. One is used with coil applicator and the other is used with drum one. This two kinds of shortwave experiments were arranged at interval of one week. Before reveiving shortwave diathermy, all subjects' skin temperatures of bilateral lower extremities were measured by Thermo tracer Detector Unit. It is a non-invasive test, the various skin temperatures were showed on a screen to be a termograph. After termination of shortwave diathermy, measured skin temperature repeatedly right after diathermy and at the following sequence interval of 30 min, 30 min, 60 min, 60 min, 60 min, 60 min. Totally follow up the obsevation of the temperature change for 5 hours after termination of diathermy. After data analysis, a similar heat effect was observed by the two different application of shortwave except the immediate effect. The immediate heat effect of cable shortwave is greater than that of drum shortwave around the tibial condyle, and the difference between them is significant statistically (p<0.05). Both lower extremities showed temperature increase significantly from right after termination of shortwave till 5 hours later even the left lower extremity (unreceiving shortwave diathermy) has been found the same phenomenon. The increase of temperature was significantly different in each leg right after ending shortwave diatherniy, the temperature increase of right lower extremity is greater than that of left lower extremity, but more than one hour later, the difference of temperature increase between them was less and became no significantly different. The extent of heat effect is from 30cm above the level of medial tibial condyle (near groin) to 25cm below the condyle level (near malleolus).
