初步比較腦性麻痺兒童使用搖桿式與雙邊滑動式操控介面操作電動輪椅之姿勢排列

Preliminary Comparisons of Postural Alignments of Children with Cerebral Palsy Using Joystick and Bimanual Gliding Access Devices to Drive the Power Wheelchair

背景與目的:對於有發展障礙的兒童來說,使用單邊搖桿式操控介面(joystick access device)操作電動輪椅可能會因為上肢不對稿的使用導致姿勢的不對稱性排列(asymmetrical postural alignments)。為了減少上肢的不對稱性使用所可能導致的不對稱姿勢排列,目前正在研發一套可以用來控制電動輪椅的雙邊滑動式(bimanual gliding)操控介面。本報告目的為初步探索腦性麻痺(cerebral palsy) 兒童使用單邊搖桿式及雙邊滑動式操控介面操作電動輪椅時的姿勢排列的可能差異。方法:共有五名腦性麻痺兒童(9.4±2.0歲)參與本報告。以修正過的坐姿姿勢控制量測量表(Seated Postural Control Measure,縮寫為SPCM)評量兒童在操作單邊搖桿式和雙邊滑動式電動輪椅時的姿勢排列。結果:與單邊搖桿式操控介面比較,五名中有兩名兒童使用雙邊滑動式操控介面表現出較佳的對稱性姿勢排列,而有兩名兒童的表現沒有差異。結論:本初步報告建議,相較於使用單邊搖桿式操控介面,使用雙邊滑動式操控介面可能有助於部分的,但非全部,腦姓麻痺兒童有較佳的姿勢排列。未來的研究需辨別適合使用此雙邊滑動式操控介面的個案。

Background and Purpose: Use of the unilateral joystick access device to control power wheelchairs for children with developmental disabilities may lead to asymmetrical postural alignments accompanied by asymmetrical functional usage of the upper extremities. An innovative bimanual gliding access device for power wheelchair was designed with the intention to promote symmetrical usage of the upper extremities and postural alignments while driving the power wheelchair. The purpose of this preliminary study was to explore the probable differences of the postural alignments of children with cerebral palsy (CP) between using the unilateral joystick and the bimanual gliding access devices to drive power wheeelchairs. Methods: Five children with CP (age, 9.4±2.0 years old) participated in this study. The postural alignments were quantified by the modified Seated Postural Control Measure. Results: As compared to the usage of the joystick access device, two out of five children demonstrated better symmetrical postural alignments while using the bimanual gliding access device, whereas two children demonstrated no difference between those two access devices. Conclusions: This preliminary report suggested that the usage of bimanual gliding access device may promote better postural alignments for some, but not all, children with CP than the unilateral joystick access device. Further study is warranted to determine the appropriate subjects who might benefit most from this innovative bimanual gliding access device.
