Foreign bodies in the airway are important emergencies in otolaryngology and most of them are non-animal. We present a 46-year-old woman who had intermittent blood-tinged sputum and a sensation of a lump in her throat for 1 week. After indirect and direct laryngoscopic examination, an animate brown foreign body was noted beneath the vocal folds. Using a direct rigid laryngoscope under intravenous general anesthesia, a 3 cm leech was removed. She often drank water directly from a spring which was possibly infested with leeches. Thus, we should keep this condition in mind in the differential diagnosis when patients present with hemoptysis or pharyngeal symptoms, especially in the eastern Taiwan.
