Time to Normalizaton of Blood Pressure after an Adrenalectomy for Patients with Primary Aldosteronism

原發性醛類脂醇過多症接受腎上腺切除後血壓達到正常之時間

摘要


研究目的:為了評估腎上腺切除手術對了腎上腺醛類脂醇瘤患者血壓之影響,我們回顧了13例在本院因腎上腺醛類脂醇瘤接受腎上腺切除手術患者的病歷。 材料與方法:總共13例原發性高醛類脂醇過多症接受腎上腺切除手術的患者。 結果:所有的患者在術前皆有高血壓,而在術後血壓均恢復至正常值,然而血壓恢復正常的時間從六日至八個月不等。我們分析了包括患者年齡、罹患高血壓之時間長短、高血壓之程度、術前醛類脂醇激素的濃度等臨床指標,發現僅有罹患高血壓之時間長短與患者術後何時血壓才能恢復正常有關連。 結論:腎上腺切除手術能有效解決這類患者高血壓之問題,然而並無臨床指標可以有效的預測其血壓恢復正常所需之時間。

OBJECTIVE: To determine the efficacy of an adrenalectomy in normalizing blood pressure in patients with aldosterone-producing adenomas (APAs), and to determine whether clinical parameters such as age, duration of hypertension, severity of hypertension, and initial plasma aldosterone levels are directly related to the time to normalization of blood pressure after an adrenalectomy. PATIENTS AND METHODS: Data were collected from 13 primary aldosteronism patients who underwent an adrenalectomy for aldosterone-producing adenomas. RESULTS: All patients were hypertensive preoperatively, and eventually, all patients became normotensive without antihypertensive medication postoperatively. The time to normalization of blood pressure ranged from 6 days to 8 months. Of all the clinical parameters evaluated, only the length of hypertension history was correlated with the time to normalization of blood pressure. CONCLUSIONS: Adrenalectomy successfully normalized the blood pressure in hypertensive patients with aldosterone-producing adenomas. However, with the exception of the length of hypertension history, other clinical parameters such as patient’s age at diagnosis, initial blood pressure, and initial aldosterone concentration could not predict the time to normalization of blood pressure after the adrenalectomy.
