Radiographically Guided Percutaneous Catheter Drainage of Empyemas
評估影像導引下經皮膿胸導管引流
摘要
於民國八十一年度內，有九個膿胸病人，接受影像導引下經皮導管引流治療。在九個病人中有八個病人的治療，主要是直接以影像導引下經皮導管引流，另一病人在傳統大直徑胸管引流失敗後才接受影像導引下經皮導管引術。全部引流是在X光透視導引下做的。我們的成功率是89%，一個失敗是因爲引流管位置欠佳所致。 在引流病人中，沒有任何嚴重併發症，我們的結論是，在具備有完善的導管植入技術前題下，影像導引下膿胸經皮膿胸導管引術是一種有效安全且可行的方法。

Nine patients with loculated empyemas receiving image-guided percutaneous catheter drainage over a one-year period were analysed. In 8 patients, image-guided percutaneous catheter drainage (PCD) was the primary treatment method; in one it was used after incomplete chest tube drainage. Drainage was performed with fluoroscopic guidance in all patients. Eight cases had successful PCD for empyemas. Only one case failed due to improper tube position. No major complications were encountered. In conclusion, with proper catheter placement, radiologically guided percutaneous catheter drainage should be safe and effective in treatment of empyemas.
