Two cases of cystic lymphangiomatosis of the spleen were presented. In the first case, computed tomography (CT) showed a markedly enlarged spleen which was almost replaced by multiple cystic, thin walled, nonenhancing lesions of varying size without cyst wall calcification. In the second case, CT scan showed a large subcapsular cystic lesion in the lower pole of the spleen with curvilinear cyst wall calcification, surrounded by several smaller satellite cystic lesions. Our two cases suggest that, despite quite different in CT scan appearance, splenic cystic lymphangiomatosis should be considered when nonenhancing thin-walled lesions in the spleen, of single or multiple low densities and varying size, are detected.
