醫學影像DICOMDIR光碟片製作流程
DICOMDIR Building-Standard Medical Image Interchange
摘要
病患自醫院所作檢查之資料，傳統係以X光膠片拷貝之。現在亦開始有病患要求以光碟片模式作儲存以利資料之攜帶或是對環保之重視。醫學影像的儲存與瀏覽必需嚴謹安全，一般常用之格式如JPEG，BMP，TIFF，GIF，PNG等因為容易被串改，所以應用在醫用影像之交換上並不安全。DICOM是一個醫學影像交換（interchange）的標準，已廣為國際醫界所接受。DICOMDIR是一個檔案的名稱，不容許被更改，但影像資料則應該可以調整視窗及中心以改變明暗度對比情形等等以利檔案之判讀。DICOM標準從第十到十二部分很清楚的界定了數位醫學影像利用可攜式媒體進行交換的架構與標準，其精髓即在DICOMDIR的產生與搭配適當可攜式媒體的應用（Media Storage Application Profiles）。本文將介紹何謂DICOMDIR，並詳細介紹高雄榮民總院的醫學影像DICOMDIR光碟片製作流程之實際過程。

There is only one choice for patients to copy their radiological studies using hardcopy films in the past. Right now they have the alternative to choose compact disc with burned medical images for more convenient portability or for the concern of environmental protection. Security and data integrity should be put into consideration regarding storage and transportation. Common image formats such as JPEG, BMP, TIFF, GIF and PNG are easily faked using some graphic tools, thus not safe enough to be distributed for medical reviewing. Images based on the common formats can not be adjusted using window width or center as DICOM does. From Part 10 to Part 12 of DICOM standard, the framework for DICOM image interchange using selected movable physical media has been clearly defined. The essence resides on the creation of DICOMDIR and proper selection of suitable movable physical media, which conform to the standard of media storage application profiles. This article will introduce the basic concept of DICOMDIR and illustrate the detailed task flow of DICOMDIR building regularly performed at Kaohsiung Veterans General Hospital. We hope that this article will enhance the understanding of DICOMDIR and promote the usage of DICOMDIR as interchange standard in Taiwan.
