Salivary duct carcinoma is a rare and highly malignant adenocarcinoma of the salivary glands. The tumors are most often located in the parotid gland and the majority of patients are male aged 65 years or older. We present a 45-year-old woman with a 2-month history of painful mass in her right preauricular region, double vision and right ptosis which had been noticeable for one month. Magnetic resonance imaging revealed a 2.4×2.7×3 cm contrast enhanced mass arising from the deep lobe of the right parotid gland extending along V3, through the foramen ovale to the right cavernous region. Deep lobe parotidectomy was performed for pathologic diagnosis, and pathologic examination showed salivary duct carcinoma. She received postoperative irradiation therapy. Unfortunately, pleural and brain metastasis were found separately on the 6(superscript th) and 13(superscript th) post-operative months, respectively, and the patient expired 14 months after surgery.
