Craniofacial resection is a standard surgical method to resect sinonasal and anterior skull base tumor. Facial incision is mandatory in traditional craniofacial resection. Postoperative scar on the face is a general aesthetic problem, and facial palsy is another possible complication. If craniofacial resection is performed using facial dismasking approach, there will be only a small circumpalpebral incision on the face. Postoperative facial scar is invisible with this approach and there is a lower risk of facial palsy. A 38-year-old female, a victim of stage 4 sinonasal squamous cell carcinoma was operated by craniofacial resection with facial dismasking flap approach in July, 2006. The major tumor mass was in the left nasal cavity and ethmoid sinus, extended into sphenoid sinus and anterior basal skull. The entire tumor was resected en bloc, followed with postoperative radiotherapy. There was no postoperative facial scar or facial palsy, and the patient was satisfied with her appearance. No tumor recurrence was seen in two and half years follow up.
