顳骨切片之鐙骨足板與顏面神經乳突段距離之計測值

背景：鐙骨足板與顏面神經乳突段之最短距離，至今未見文獻報告。本研究擬計測顳骨切片之顏面神經乳突段與鐙骨足板之最短距離。顏面神經乳突段最靠近鐙骨足板者，由於顏面神經走向之故，理應在最靠近錐隆處。材料與方法：本研究取測11組成人顳骨切片，以Aperio ImageScope計測顏面神經乳突段至鐙骨足板之最短距離，結果以學生t-test統計分析。結果：經取測的11組顳骨切片中，顏面神經乳突段至鐙骨足板前緣之最短距離為3.032 ~ 4.791（3.904 ± 0.777）mm。顏面神經乳突段至鐙骨足板後緣之最短距離為0.867 ~ 3.691（2.385 ± 0.895）mm。結論：顏面神經乳突段至鐙骨足板前緣後緣之最短距離在4.791 mm至0.867 mm之間，平均未超過5mm。在施行鐙骨及鐙骨相關手術時，顯微器械可回轉之空間十分有限，宜謹慎操作。

BACKGROUND: Since there is no available data concerning the shortest distance between the stapedial footplate and the mastoid segment of facial nerve, this research was designed to detect this distance. The shortest distance between the stapedial footplate and mastoid segment of facial nerve should approach the pyramidal eminence due to the alignment of facial nerve.METHOD: Eleven sets of adult human temporal sections were enrolled in this study. The shortest distance between the stapedial footplate and mastoid segment of facial nerve were measured by Aperio ImageScope. Data was evaluated by student t-test.RESULTS: The shortest distance between anterior border of stapedial footplate and mastoid segment of facial nerve was 3.032~4.791 (3.904 ± 0.777) mm. The shortest distance between posterior border of stapedial footplate and mastoid segment of facial nerve was 0.867~3.691 (2.385 ± 0.895) mm.CONCLUSIONS: The shortest distance between mastoid segment of facial nerve and the anterior and posterior borders of footplate ranges from 4.791 mm to 0.867 mm, suggesting that there is only available space of less than 5 mm for operating between the mastoid segment and footplate. Great care should be taken during operating this area.
