Button battery as an external auditory meatus foreign body complicated with tympanic membrane perforation is rare. One 8-year-old child was referred from emergency room with severe otalgia and a button battery foreign body in the right ear for 3 days. Under topical anesthesia with 4% xylocaine spray, an alkaline button battery (LR41) was removed. Congestion of the ear canal and a small perforation on upper right tympanic membrane with diffuse peripheral myringitis were observed. Topical neomycin ointment and oral antibiotics and analgesics were administered. Tympanic membrane perforation healed one week later, and pure tone audiogram reported normal hearing level. The patient remains symptom free after six-month follow-up.
