Endoscopic Electrocauterization of Pyriform Fossa Sinus Tracts

BACKGROUND: Pyriform fossa sinus tracts are rare congenital head and neck entities. Conventional treatment includes surgical resection of entire sinus tracts. However, surgery is complicated and invasive, particularly in younger patients; hence, endoscopic cauterization has become increasingly popular in these patients. METHOD: We investigated the effectiveness and safety of this procedure. Four patients (age, 3-11 years) with pyriform fossa sinus tracts treated by endoscopic electrocauterization at the Kaohsiung Veterans General Hospital from January 2011 to December 2016 were retrospectively analyzed. RESULTS: All patients underwent endoscopic electrocauterization, with two procedures performed as revision surgeries. Only one patient who underwent exploratory neck surgery before the procedure suffered temporary vocal palsy. During the median follow-up of 893 (range, 711-1280) days, no recurrence occurred in any patient. CONCLUSION: Endoscopic electrocauterization is a safe, minimally invasive, and effective alternative to conventional open procedures for pyriform fossa sinus tracts and can be applied during primary and revision surgeries in younger and older patients.
