視網膜剝離手術－內置扣壓與外置扣壓之比較
Retinal Detachment Surgery Comparison of Implant and Explant
摘要
晚近十五年來，視網膜剝離手術的扣壓術(Buckling procedure)主要分為兩大方式。一為鞏膜內置扣壓術(Scleral implant)，合併透熱療法(Diathermy)。一為鞏膜外置扣壓術(explant)合併冷凍凝固法(cryopexy)。本文著者在榮總眼科，自民國66年7月至民國69年8月，收集視網膜剝離手術之患者，除去資料不全者後，計得134例。66年7月至67年12月，多數病例採用鞏膜內置扣壓法。68年1月至69年8月，則大多採用鞏膜外置扣壓法。比較兩者手術時間、結果及併發症後，著者認為兩法各有其優點。但是採用鞏膜外置扣壓法者，平均手術時間較短，術後視力之恢復比較明顯。

The author compared 69 cases of implant scleral buckle and 65 cases of explant scleral buckle for the repair of retinal detachment. Implant means the silicone material was embedded in the dissected scleral pocket and this procedure combined with surface diathermy on the scleral bed. Explant technique means direct suturing of the silicone material on the sclera and combined with cryopexy. In comparing the results of both techniques, the author favors explant technique. In general, the explant technique has shorter operation time and higher success rate. The vitreous is relatively clearer in the immediate post-operative period in the explant group and hence is better for the observation of fundus conditions. In regard to the complications, there was no obvious difference among the two groups.
