重度子癇前症併發兩側視網膜剝離及視神經乳頭水腫－病例報告

Severe Preeclampsia Complicated with Bilateral Exudative Retinal Detachment and Edema of the Optic Disc

摘要

一名二十四歲女性病人，懷孕第二十七週時發生子癇前症，有高血壓，水腫，以及蛋白尿，由於無法控制之高血壓及持續惡化之腎功能障礙，在三十週時給她做了剖腹生產，隔天病人開始出現視力模糊的症狀，眼底檢查發現兩眼都有很高的大泡性視網膜剝離及視神經盤水腫。五天後，視網膜完全貼回去，視力逐漸恢復。眼底可以看到高血壓視網膜病變（動脈攣縮，網膜出血，棉花狀斑，以及局部網膜內動脈旁漏出物FIPTs），高血壓脈絡膜病變（滲出性視網膜剝離，脈絡膜梗塞，以及色素網膜變性），及高血壓視神經病變（表現視神經盤水腫）。

A 24 y/o female patient with pregnancy at 27th week developed preeclampsia with hypertension, edema, and proteinuria. Cesarian section was performed at the 30th week due to uncontrollable hypertension and progresive renal dysfunction. Blurred vision developed on the next day, and bilateral high bullous retinal detachment with edema of the optic discs were noted. The retina reattached 5 days later and VA gradually improved. The fundus showed hypertensive retinopathy (arteriolar constriction, hemorrhage, cotton wool spots, and focal intraretinal periarteriolar transudates), hypertensive choroidopathy (exudative retinal detachment, choroidal infarction, and RPE degeneration), and hypertensive optic neuropathy(optic disc edema).
