脈絡膜骨瘤之病例報告―兼論螢光眼底攝影在類似病灶之診斷價值

摘要

男性，59歲之商人，因訴左眼視力模糊而求診檢查時，右眼視力1.0，左眼視力0.1，兩眼眼壓正常，眼底鏡檢查，左眼在顯上側離開視神經乳頭5mm處一有4.5×6mm之黃白色境界鮮明之瘤塊。經螢光眼底攝影，電腦斷層掃描，超音波檢查確定為脈絡膜骨瘤。作者並收集另外三例類似脈絡膜腫瘤（其中二例病理切片檢查已確定為黑色素瘤），評估螢光眼底攝影在診斷類似病灶之價值。

A yellowish-White, well-defined choroidal osteoma was found in the left eye of a 59-year-old man. The diagnosis was established by a series of examinations including the echogram, computerized tomogram, and FAG. etc. Clinically, choroidal osteoma should be differentiated from choroidal melanoma. We collected three other cases of simulated choroidal melanoma, of them two were melanoma proved pathologically. In all the cases except one, FAG showed early hyperfluorescence, diffuse extensive leakage, and late tumor staining. Therefore, the diagnostic value for FAG in the simulating choroidal lesions could only be said as ”compatible” or ”incompatible”.
