摘要


本篇報告以杜卜勒彩色超音波偵測在用力時的伐氏操作(Valsalva maneuver)所出現的眼部血液動力變化。發現不但眼窩內的靜脈血流會出現減緩甚至逆行的現象,眼動脈的各個分枝上也出現立即的變化:包括每脈動之最高血液流速、最終血液流速、平均血液流速、以及血液流速積分的各項數值約隨著胸腹壓力的增加而有顯著的提高。由此,我們討論Valsalva hemorrhagic retinopathy形成的原因,並認定用力的伐氏操作時動脈血流增加對於視網膜血管所造成的衝擊,是眼部出血症狀發生的主要機轉。

In this study, we assessed the ophthalmic hemodynamic changes during Valsalva maneuver by non- invasive color doppler ultrasound. It demonstrated that the retrobulbar venous flow velocity was diminished or reversed during increased thoracic-abdominal pressure. However, we found that there was a more prominent change in the ophthalmic arterial flows during the straining. With the onset of the maneuver, all of the peak-systolic flow velocity, end-diastolic flow velocity, mean-enveloped flow velocity, and flow velocity integral of the anterograde ophthalmic arterial flow increased significantly. These facts support our hypothesis that the increased ophthalmic arterial flow may also be responsible for the ophthalmic hemorrhagic sequelae of the Valsalva maneuver.
