雙側性眼球自剜－壹病例報告
Bilateral Autoenucleation: A Case Report
摘要
眼球自剜(autoenucleation)，是種極罕見且最嚴重的眼睛自我傷害(self-mutilation)，且被認為是精神疾病的一種相當特殊的表徵。國外文獻曾報導過，不論是眼科或精神科的著作，其所肇因的精神疾病，皆以精神分裂症最多，其次是憂鬱症、躁症、強迫症、藥物濫用以及器質性腦症等。然而，仍有少數病人找不到確切的精神病學上的診斷。本文所要敘述的，乃是一個在精神醫學上並不具有典型精神病症狀的病人，其發病及其後的治療經過，以及過去文獻之回顧與分析。

Autoenucleation, or oedipism, the most extreme form of ocular self-mutilation, is an uncommon and specific manifestation of psychiatric illness. The associated psychosis may be due to schizophrenia, drug abuse, obsessive/compulsive neuroses, depression, manic phases of bipolar mood disorders, organic mental disorders, and so on; however, there are still a few cases without definite psychiatric diagnosis. We reported a thirty-nine years old female who, during an acute episode of presumed atypical psychosis, committed incomplete autoenucleation of both eyes. The mental status of the patient and surgical management are discribed; together with the review of literature.
