一個四歲小女孩從出生後即發現右眼有往上偏位的現象,未接受過任何眼科手術,最初在門診依右上斜肌及右下直肌麻痺之診斷入院準備接受垂直肌手術。在手術過程中意外地發現沒有右下直肌,於是將右下斜肌切除之後,隨即把內外直肌半移位至六點鐘方向距離角膜緣六毫米處。術後眼位回復正常,結果令人滿意。眼外肌肉之缺無在文獻上相當罕見,本文提出一先天性無右下直肌之病例報告並探討其手術方法。
A 4-year-old girl was noted to have upward deviation of right eye since birth. She had no previous history of eye surgery. At our clinic, right superior oblique muscle and right inferior rectus muscle paralysis were suspected and vertical muscle surgery was planned. During the operation the right inferior rectus muscle was found to be absent incidentally. We then performed hemitransposition of horizontal muscles to the supposed inferior rectus muscle insertion site after doing right inferior oblique muscle myectomy. Postoperative result is satisfactory and eye position is parallel. Absence of the extraocular muscles is a rare entity which has been reported only sporadically in the literature. This report illustrates a case of congenital absence of the inferior rectus muscle.
