本研究目的在建立護理人力生産力指標,並以生産力指標來評估各單位工作績效及人力分配情形,以做爲工作攷核、人力資源分配及提供全民健保項護理活動收費參攷。本研究人力生産力指標是依工時測試護理人員實際給予病患的護理時數爲輸出,除出護理人員實際排班之工作時數爲輸入所得的商數,生産力在1加減0.2爲可接受之範圍。調查霎時間自民國八十年八月起至八十一年七月止,共計一年。研究對象爲某一級教學醫院中內外科、婦産科、小兒科、精神科及加護房病等七十二個單位。研究方法採護理人員自我記錄及觀察法,調查269項各項護理活動平時工時及頻率(含共同護理活動、單位相關護理活動及各專科護理活動),以此計算護理時數及建立人力生産力指數。調查活動信度採觀察員信度,效度採專家效度。研究結果顯示:某一級教學醫院護理人力生産力指數爲1.20,各組別人力生産力指數內科組爲1.03,外科組爲1.10,加護組爲1.48,婦兒科組爲1.02,精神科組爲0.61,綜合科組爲1.28,各組因科別性質不同而有個別差異。研究結果可提供各病房各項護理活動所需平均工時、發生頻率、護理進時數、各項活動護理收費、護理人力生産力指數外,並可提供護理部做爲分配工作、績效資金、人力資源分配及全民健保護理收費之參攷。
The purposes of this study were to establish indicators to assess nursing manpower productivity and applied these indicators to evaluate the nurse’s performance and to determine the number of nurses to meet the work. The research started on August 1,1991 and ended on July 31, 1992. Seventy-three units including the medical-surgical, obstetrical-gynecological, pediatrical and psychiatric units had participated in this study. A total of 1,440 nurses and co-workers were involved. Self-report and partially direct observation methods were used to record the mean time and frequency of 269 items of nursing activities per patient per day. These 269 items were classified into general nursing activities, specific nursing activities and related activities. Tests for interrater reliability were used. Validity was based on experts’ opinions of content validity. The results showed that the nursing manpower productivity index for this hospital was 1.20, 1.30 for medical units, 1.10 for surgical units, 1.48 for intensive units, 1.02 for obstetrical-gynecological units, 1.02 for pediatric units, 1.28 for medical-surgical units, 0.61 for psychiatric units. There were significant differences between thedical and psychiatric units. The research helped establish the mean time and frequency of these 269 items of nursing activities. The basic data for nursing activity time, frequency, nursing hours and nursing productivity indicators helped evaluate nurses’ performance and workload.
