成人急性白血病病患生活品質及其相關因素之探討

摘要

本研究爲描述性橫斷面的研究，目的在解成人急性白血病病患經化學治療後達病情完全緩解的生活品德，並探討病患個人基本資料、疾病資料、疾病狀況、症狀困擾與生活品質的關係。研究工具採生活品質量表、症狀困擾量表。於八十三年二月初至八十三五年月中旬，以立意取樣方式在台北地區四所醫學中心血液腫瘤科門診，選取66名成人急性白血病病患爲研究對象，所得資料以百分比、平均值、皮爾森積差相關、無母數Kruskal-Wallis單因子變異數分析等方法進行統計分析。研究結果顯示：(1)66名病患皆有中度以上的生活品質，其中以「家庭因素」得分最高，「心理社會因素」得分最低；(2)個人基本資料方面：以「婚姻狀況」、「職業狀況」、及「更換工作」及「自己或配偶所提供」的「經濟支持」等三變項分別與生活品質各構面及整體生活品質，有統計上的顯著關連；(3)病患的疾病狀況與生活品質無統計上的顯著關連；(4)病患所遭受的「症狀困擾」與「生活品質」吳負相關。經由研究結果可提供護理人員在照顧此類病患時，不但要注重病患病情的緩解，更要注重病患生活品質的提昇，以及持續性的提供症狀之護理。

This descriptive cross-sectional study was undertaken to investigate the quality of life and its correlated factors in caring for adult acute leukemia patients. The studied variables included individual characteristics, disease condition, and symptom distress. In this study, sixty-six adult acute leukemia patients were selected from four hematological oncology clinical centers in Taipei. The Quality of Life Index (QLI) and symptom distress questionnaires were used to assess quality of life. The results showed (1) the overall QLI was 21.83, indicating moderate quality of life. In each dimension of QLI, the highest was “family dimension” and the lowest was “psychosocial dimension”; (2) as for individual characteristics, marital status, employment status (job changing) and economic resources (single, couple) of acute leukemia patients were correlated with part or whole quality of life; (3) relative to the disease condition related, no statistically significanct correlation was found among 66 adult acute leukemia patients. (4) Symptom distresses among the 66 adult acute leukemia patients were negatively correlated to whole quality of life. The results suggested that outcome of medical treatment, quality of life and symptomatic care are vital to nursing care of adult patients with acute leukemia.
