Twenty-eight segmental fractures of the diaphysis of the femur were treated with Grosse-Kempf interlocking nailing. All of the fractures had been caused by trauma induced by high-speed traffic accidents. There were 22 closed and 6 open fractures. Among these cases, 12 were double-level fractures of the middle frag,ent, which are so-called split segmental fractures. During a 4-year period, 28 operations of static interlocking nailing for femoral segmental fractures were performed at our trauma team by 4 surgeons. There were 5 delayed unions and 2 malunions. Virtually all fractures united in the average period of 33 weeks, regardless of the pattern of the fracture or the degree of comminution. Closed static interlocking nailing is the treatment of choice for most segmental fractures of the shaft of the femur.
