A falling metal bar penetrated the right occipital are of a 22-year-old man. He remained coherent and in a stationary condition on arrival at the hospital. No other injury was found. After stabilization, he underwent an emergency operation. Brain injuries and superior sagittal sinus penetration were repaired. Massive hemorrhave occurred during vascular repair; however, he survived with homonymous hemianopia. Penetration head trauma by a blunt object is rare. The location and penetration tract determine the extent and kind of injuries. Patients mostly die form brain injury or hemorrhage; however, some may arrive neurologically intact at the hospital. There is a chance to save a life if qualified care can be provided in time.
