台北市立醫院門診用藥分析

摘要

本研究主要藉由台北市政府所屬醫療院所向中央健康保險局申報的資料庫中，分析市立醫事服務機構門診處方箋型態。研究期間自１９９９年７月至１９９９年１２月，研究方法係採用檔案分析法，以藥品為分類對象，經統計結果，有效申報案件1,179,372筆，有效申報案件的總申請金額為1,386,125,526元，平均每一申報案件的申報金額是1,175元。所有特約醫事機構的治療明細檔資料分析結果，全部藥費申報金額共是711,832,682元，其藥費佔所有醫療費用的５１．４％平均每張處方的藥品金額是６０４元，平均每日藥品金額是53.46元，平均給藥天數是１４天，而醫令檔資料顯示平均每張處方的藥品品項是３．５種。　藥品使用之分析，若以藥品的藥理分類來分析，藥品處方使用頻率的狀況，則以「腸胃道用藥」、「中樞神經系統劑」、「心臟血管劑」、「荷爾蒙及其相關合成製劑」以及「自主神經劑」依序排行前五名。藥品使用金額統計結果、則以「心臟血管劑」、「中樞神經系統劑」、「腸胃道用藥」、「荷爾蒙及其相關合成製劑」以及「抗感染劑」依序排行前五名。

The research was intended to understand the status of medicine utilization in Taipei public hospitals. We collected and analyzed the data of six months consumption of all medicines in terms of the prescription patterns in the ambulatory patients under nine public city hospitals during July and December 1999. Which consists of 1,179,372 valid claimed cases from the detailed therapeutic files. The total medical claims was NT$1,386,125,526. an average of NT$1,175 per case. Detailed Therapeutic File analysis indicates a total consumption of NT$711,832,682. It represents approximately 51.4% of the total medical expenditures. The average drugs cost per case is NT$604 and NT$53.46 per day. The average length of prescribed drugs is 14 days per prescription and the average items of medicines per prescription is 3.5. In terms of pharmacology classification, ‘central nervous system agents', ‘cardiovascular drug’ , ‘anti-infective agents’ , ‘gastrointestinal drugs’ , and ‘autonomic drugs’ are the five most frequently prescribed medicines accordingly. The top 5 categories in terms of drug cost were ‘cardiovascular drugs’, ‘central nervous system agents’, ‘gastrointestinal drugs’, ‘hormones and synthetic substitutes’, and ‘anti-infective agents’.
